Driver of incoming train and guard at leading end of the reversing train killed. Driver of reversing train and guard of incoming train injured. Signaller set route and cleared signal for reversing train which did not pass signal after 5 minutes. Signal for the reversing train was reset to danger and a new route set for incoming train. Reversing train started off against the signal and collided with incoming train. Deceased guard held responsible for not keeping adequate lookout. Signaller partly responsible for not checking driver of reversing train knew the signal had been reset to danger.

The stopped train was then run into by a following postal express hauled by LMS Coronation Class 4-6-2 No 6251 City of Nottingham.
